About The Position

Cornerstone Family Healthcare is actively recruiting for a Peer Navigator to join our growing team in Poughkeepsie. This is a part-time position with a maximum of 21 hours per week. The Peer Navigator will support the HCV Patient Navigator in providing screening, testing, and linkage to care for clients within Dutchess, Orange, and Sullivan Counties. Cornerstone Family Healthcare is a non-profit Federally Qualified Health Center dedicated to providing high-quality, comprehensive primary and preventative health care services with dignity and respect to all individuals, regardless of their ability to pay. For over fifty years, Cornerstone has served the community, with a focus on the underserved and those lacking access to healthcare.

Requirements

  • A High School Diploma
  • Experience in peer education, HCV counseling and/or disease prevention.
  • Familiarity with the target population and the geographic area assigned.
  • Commitment to undergo training about HIV/HCV/STI, harm reduction, mental health and community resources.

Responsibilities

  • Participate in outreach to individuals at high risk at the community level as well as within drug rehabilitation centers, shelters, soup kitchens and other community-based organizations in order to publicize program services and recruit potential clients.
  • Participate in outreach to community partners.
  • Provide referrals and referral follow-up to clients for needed services when applicable.
  • Work with the HCV Patient Navigator in referring clients for testing, linkage to treatment and other services as deemed appropriate.
  • Observe and abide by HIPAA Law and agency confidentiality standards.
  • Assist the program staff in promoting and implementing program services.
  • Pass on to the HCV Patient Navigator, any referrals for HCV testing and counseling.
  • Assist program staff in conducting outreach recruitment activities through the use of Social Media, presentations to services providers and networking with community organizations and key community members to reach out to individuals at high risk.
  • Provide clients with reminder phone calls for appointments, engagement and re-engagement efforts, transportation, escort and other supportive services.
  • Complete the mandated continuing education trainings as selected and scheduled by the Program Supervisor.
  • Assist in program coverage when required.
  • Attend and participate in monthly department, All Staff and other required meetings.
  • Be familiar with Cornerstone policies and procedures and the Employee Handbook.
  • Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
  • Perform other related duties as assigned.
