About The Position

Transitional Services for New York, Inc. is seeking a full-time Peer Health Navigator for its Behavior Health Care Coordination (BHCC) program in Jamaica, Queens, NY. The position works with care coordination members to develop recovery plans and support effective management of future crises. The Peer Health Navigator conducts outreach and enrollment and connects members with community-based resources addressing physical health, behavioral health, and social determinants of health. The role is part of a care coordination team supporting individuals experiencing behavioral, medical, and economic crises.

Requirements

High school diploma or equivalent.
NYS Peer Certification.

Responsibilities

Provide peer counseling.
Complete and implement peer health navigation training.
Help members identify and connect with community resources, including vocational programs, outpatient programs, psychosocial clubhouses, and health care services.
Assist care coordinators with tracking and monitoring members’ attendance at linked services and support services.
Escort members to appointments as needed.
Communicate with members’ networks to monitor service needs.
Conduct outreach to members and referral sources to educate them about care coordination services.
Visit members in the community to enroll them in care coordination services.
Document all encounters in progress notes using electronic health platforms.
Attend staff meetings and special on-site and off-site trainings, seminars, and conferences.
Support members in developing a WRAP Plan.
Complete HRA2010E housing applications and support members during interviews and tours.
Travel independently by public transportation.
Assume and adhere to assigned Job Action roles under the Disaster Preparedness Plan during a declared disaster.
Perform other related duties as required.
