About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models. JOB SUMMARY A Peer Based Service Coordinator is an individual who is in recovery from substance use and/or general mental health for a minimum of six months and assists in addressing all of the ancillary concerns and needs of the participant in conjunction with the established treatment team. This position is responsible for ensuring the coordination of care with all those parties who are currently involved in the patient’s case, ensuring that family and other social supports identified in the patient’s treatment plan are involved in the treatment planning process (when appropriate), as well as addressing some of the DSMV; Avis IV stressors. The Peer Based Service Coordinator works in conjunction as part of a treatment team made up of a Clinical Supervisor, Licensed Behavioral Health Professional, Outpatient Outreach PSS and other involved stakeholders in the participants’ treatment. Each area performed will be under the direct clinical supervision of either a behavioral health professional as identified in A.A.C. R9-20-101.15 or a behavioral health technician as identified in R9-20-205.B.2.

Requirements

  • High School Diploma or GED required.
  • Minimum of six (6) Months of experience in the behavioral health field required.
  • Minimum of six months of recovery from substance use and/or mental health disorders required.
  • Certified Peer Support Specialist (CPSS) required
  • Current AZ Driver’s License or equivalent of state of residence.
  • Clear 39-month Motor Vehicle Record required.
  • Fingerprint Clearance Card required.

Responsibilities
