About The Position

We are seeking a compassionate, patient-centered Ambulatory Physician to join our growing outpatient care network. As an Ambulatory Physician, you will provide comprehensive outpatient acute medical care, primary preventive health, and partner with a multidisciplinary team dedicated to achieving exceptional patient outcomes for children from birth to young adulthood. Team members that support your in-clinic work include nurse case managers, social workers, registered dietitians, nurses, medical assistants, and other healthcare professionals who work together to support coordinated, holistic care for our patients. This role offers the opportunity to deliver high-quality, evidence-based care in a collaborative environment focused on improving patient access, clinical outcomes, and the overall care experience for patients and families.

Requirements

MD/DO
Unrestricted medical license in Illinois
Board certified pediatrician required
Post residency
2-3 years of experience
Excellent written and verbal communication skills
Excellent interpersonal skills
Demonstrate knowledge and expertise in patient care (pediatrics preferred)
Active member in appropriate professional organization
CPR issued by the American Heart Association or the American Red Cross.

Experience with medically complex and technology dependent patients is appreciated
Bi-lingual Spanish preferred

Responsibilities

Utilizes advanced knowledge, skills, and expertise in pediatrics for an identified population of patients, including acute & primary care, health promotion, prevention of disease, and management of chronic disease.
Performs delegated medical functions as described in written protocols and/or a collaborative agreement that have been approved by medical and nursing administration.
Systematically assesses the physical, psychosocial, and developmental needs of patients and families through the use of a medical screening exam (MSE), history and physical exam, and other assessments as appropriate.
Develops a plan of care for each patient/family based on assessment and documents according to policy.
Demonstrates competence consistent with evidence-based practice in assessment, planning, implementation, and evaluation of identified patients.
Monitors clinical outcomes to improve the quality of care for patients.
Evaluates and modifies the plan of care as needed.
Integrates the role of teacher into clinical practice to provide patient education.
Assesses learning needs, provides for education and evaluates learning objectives are achieved.
Assists the staff in providing patient and family education.
Consults within the organization on current pediatric clinical practice and disease management in area(s) of expertise.
Functions as a resource to the community in areas of expertise.
Participates as a member of related professional organizations.
Functions as a role model and informal leader.
