Requirements

Master's degree in Speech Language Pathology required
Current Vermont State SLP license required
Two years of clinical Speech Language Pathology experience required
Active enrollment in clinical fellowship year or successful completion of certificate of clinical competency required
Valid Driver's License

Responsibilities

Evaluate, plan, and administer medically prescribed or educationally relevant speech therapy treatment to clients in their homes or students in their respective educational environment.
Provide clinical supervision of students and SLPs in their CFY year and students.
Actively monitor and amend patient visit schedule to maximize productivity and patient need, accounting for new patients and changes in existing schedules.
Perform comprehensive (OASIS and SLP) assessments of assigned patients, including psychosocial, educational, safety, and risk factor parameters.
Develop a plan of care to promote optimal functional independence and safety.
Plan and provide skilled speech-language therapy treatment, including therapeutic techniques and patient/caregiver education, assessing patient status and progress.
Coordinate patient care with other clinical team members, including physicians, nurses, caregivers, other rehab services, and students.
Perform functions related to school-based services, including evaluations, documentation, development of Individual Educational Plans (IEPs), participation in meetings, and provision of direct and consultative services.
Complete insurance paperwork.
Assess patients' needs for HCA services, develop and coordinate plan of care with patient/caregiver, and provide ongoing supervision.
Coordinate safe patient discharges and/or transfers and ensure all appropriate paperwork and patient information is processed.
Serve as a Home Care ST as required.
