About The Position

We are seeking an experienced and compassionate Pediatric Registered Nurse (RN) Supervisor to join our Certified Home Health Agency (CHHA) team. In this leadership role, you will oversee and support nursing staff while ensuring the delivery of high-quality, patient-centered care to pediatric patients in the home setting. The ideal candidate has a strong clinical background in acute pediatric hospital settings, along with leadership or supervisory experience. CHHA experience is a plus.

Requirements

  • Active Registered Nurse (RN) license in New York State
  • Acute pediatric hospital experience required
  • Minimum 2 years of pediatric nursing experience required
  • Prior supervisory or leadership experience preferred
  • CHHA experience is a plus
  • Strong knowledge of OASIS and home health regulations preferred
  • Excellent communication, organization, and leadership skills
  • Valid driver's license and ability to travel to patient homes
  • Ability to travel to patient homes, including walk-ups and varied environments
  • Ability to stand, walk, bend, and lift as needed
  • Ability to lift up to 50 pounds unassisted and more with assistance
  • Pre-employment and annual health assessments
  • Compliance with all New York State immunization and health requirements

Nice To Haves

  • CHHA experience is a plus
  • Strong knowledge of OASIS and home health regulations preferred

Responsibilities

  • Provide clinical supervision and guidance to field nurses caring for pediatric patients
  • Conduct patient assessments, including admissions and supervisory visits
  • Ensure individualized plans of care are appropriate, updated, and followed
  • Monitor patient progress and intervene as needed to ensure optimal outcomes
  • Collaborate with physicians and interdisciplinary team members
  • Supervise, mentor, and support RN staff in the field
  • Conduct performance evaluations and provide ongoing feedback
  • Assist with orientation and training of new nursing staff
  • Serve as a clinical resource for complex pediatric cases
  • Coordinate care across interdisciplinary teams, including therapists, aides, and physicians
  • Participate in case conferences and care planning meetings
  • Ensure services are delivered according to physician orders and agency standards
  • Review and ensure accuracy of OASIS documentation and clinical records
  • Maintain compliance with state, federal, and agency regulations
  • Oversee timely completion of all required documentation
  • Participate in rotating on-call coverage
  • Provide clinical support and guidance during on-call hours
