About The Position

Community Health Center Inc. is seeking a full-time Pediatric Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join their team at Connecticut Pediatrics @ CHC in Hartford. This role involves providing high-quality, comprehensive nursing care to individuals and families. The RN will work at the top of their licensure to enhance care safety, effectiveness, and efficiency, collaborating with primary care providers to coordinate care, support transitions, and educate patients. The LPN will provide clinical and technical support to primary care providers, implement planned care using electronic tools, monitor equipment, and liaise with patients, families, and the healthcare team. Both roles emphasize patient-centered care within a medical home model and adherence to infection control standards.

Requirements

Registered nurse, in good standing.
Current CT Nursing License
Current American Red Cross CPR for the Professional Rescuer and AED (CPR/FPR/AED) certification or Current American Heart Association BLS/AED for Healthcare Providers certification.
LPN Program Completion Certificate
Skills include EKG use, appointment scheduling, word processing and computer skills, and familiarity with medical testing procedures.
Sound organizational skills and accuracy at all levels of job.
Prior demonstrated excellent communication and customer service skills.
Must be team focused and demonstrate leadership qualities.
Current CT LPN License
CPR—American Red Cross CPR for the Professional Rescuer and AED certification or American Heart Association BLS/AED for Healthcare Providers

Responsibilities

Support primary care of patients under planned care model, including pre-visit planning, daily huddles, and execution of nurse-led and nurse-assisted visits.
Collaborate with PCP to identify needs for preventive care, evidence-based chronic disease care, and care coordination.
Provide general and specific health care education during patient visits and by telephone.
Work with patients to identify and monitor self-management goals.
Collaborate and coordinate care with other community-based providers.
Follow up on emergency room visits and hospital discharges.
Assist providers with exams, treatments, and procedures as needed.
Ensure safe, accurate, clinically appropriate collection, storage, and processing of all biologic specimens.
Administer medications, including immunizations, at the order of prescribing provider and under standing orders.
Monitor patient response to medication, detect, report, and respond to any adverse reactions, and reconcile medications.
Perform a wide range of nursing activities as needed and after initial prescription by primary care provider such as wound care, catheterization, etc.
Accurately document all aspects of patient care in the medical chart on a timely basis.
Schedule tests and referrals for specialty care on order of provider.
Maintain strict infection control standards in accordance with CHCI protocols and universal precautions TJC, OSHA and CLIA standards.
Act as a resource person for medical assistants regarding patient preparation, needs and procedures needed.
Monitor and ensure follow-up of all abnormal pap smear results, STDs, lead screens and other tests and screens performed at CHCI.
Oversee the maintenance of supplies and equipment, ordering supplies as necessary.
Maintain compliance with agency mandatory annual training (Patient Safety, Abuse, Age Based Competencies, EOC, Abuse trainings, etc.).
Utilize electronic health record for timely, safe, confidential storage and retrieval of patient information.
Participate in Project ECHO complex care management to continually advance knowledge and skills in care management and care coordination.
Present nursing information and insight into patient care with other members of the care team at Integrated Care Team meetings.
Implement all planned care items using electronic tools to ensure efficient and accurate workflow.
Monitor equipment and perform clinical testing according to OSHA and CLIA standards.
Liaise with patients, families, specialists, and all members of the health care team.
Ensure continuity and coordination of care in a patient-centered medical home.
Assist the primary care nurse as able with routine medication administration, immunizations and other provider clinic support supported by the scope of LPN licensure.
Apply evidence-based guidelines from major regulatory and specialty groups, perform pre-visit preparation for all primary care patients scheduled to be seen.
Collaborate with the primary care nurse and provider to 'e-huddle' utilizing the Planned Care Dashboard to ensure complete and thorough primary care visits.
Prepare patients to be seen by provider, using established criteria as appropriately determined by the nature of the visit.
Accurately document chief complaint and results from evidence-based screening tests.
Accurately perform and document patient vital signs.
Independently and accurately perform appropriate CLIA waived tests, EKG, hearing & vision test.
Prioritize work responsibilities on a daily basis, decreasing waiting time.
Collaborate with providers, clinical and clerical support staff to facilitate appropriate patient flow.
Effectively work as a member of the health care team assisting other team members as needed, or requested.
Appropriately schedule tests and referrals for specialty care in accordance to Provider orders in a timely manner.
Maintain a clean, orderly, safe working environment.
Oversee maintenance of supplies and documents in medical logs as directed.
Set up and stock exam rooms and clean rooms at end of each day worked.
Adhere to all infection control standards, follow Universal Precautions and safety policies and procedures of the Agency.
Maintain yearly CPR certification as required for all clinical employees.
Exhibit excellent customer service skills to external and internal customers at all times with a focus on quality of care and teamwork that focuses on the needs of the department and/or the business in conjunction with the full mission of the Agency.
Comply with Agency mandated in-service training workshops (EOC, Working Safely, Abuse, etc.).
Assist the primary care nurse with provider clinic support such as routine medication administration, immunizations, and other items as able.
Administer all medications and immunizations under the delegation of CHCI’s standing order, or Primary Care RN or Primary Care Provider delegation.
