We are seeking a dedicated Registered Nurse (RN) Pediatric Case Manager to provide one-on-one home health visits throughout the Tucson area. This is an excellent opportunity for an RN who is passionate about building meaningful relationships with pediatric patients and their families while enjoying the autonomy of managing a consistent caseload within a defined service area.

Requirements

Current Arizona Registered Nurse (RN) license in good standing
Valid driver's license, reliable transportation, and current auto insurance
Strong assessment, communication, and critical thinking skills
Excellent organizational and time-management abilities
Passion for providing compassionate, family-centered pediatric care
Current BLS certification

Nice To Haves

Pediatric, home health, or case management experience preferred, but not required

Responsibilities

Provide skilled nursing care to pediatric patients in their homes
Develop, implement, and oversee individualized plans of care
Perform comprehensive patient assessments and ongoing evaluations
Coordinate care with physicians, therapists, caregivers, and other members of the healthcare team
Educate patients and family members on disease management, medications, treatments, and home care
Monitor patient progress and communicate changes in condition promptly
Complete accurate and timely clinical documentation in accordance with state, federal, and agency guidelines
Ensure the highest standards of patient safety, quality care, and family-centered service
