About The Position

Affirmed Home Care is seeking an experienced, compassionate, and motivated Registered Nurse (RN) Care Manager to support pediatric clients throughout Bronx County, NY. Requirements

• Minimum of 2 years of recent RN experience in pediatric home care or acute care
• Strong pediatric clinical experience with: Ventilator and tracheostomy care, G-tube/PEG tube care, IV infusions, Wound care management
• Active New York State RN License
• Current BLS Certification
• Recent physical exam (within 1 year)
• Current PPD/QuantiFERON (within 1 year) or chest X-ray (within 5 years)
• Valid MMR lab report
• Valid driver’s license and reliable transportation
• Authorized to work in the United States

Responsibilities

• Conduct in-home nursing assessments and case management visits
• Develop, implement, and oversee individualized pediatric care plans
• Obtain and maintain physician orders in accordance with NYS regulations
• Monitor, document, and communicate changes in patient condition
• Perform supervisory visits for home care staff
• Collaborate with families, physicians, and interdisciplinary care teams
• Promote patient safety, comfort, independence, and continuity of care
