About The Position

Under the general supervision of the Healthy Steps Program Supervisor and in coordination with the Director of Pediatrics and Program Manager, the Pediatric Patient Health Worker provides childhood development expertise within the pediatric primary care team at Salud Para La Gente. This role involves providing screenings, interventions, requests for referrals, and follow-up for families with pediatric patients. The position focuses on building strong relationships with families and providers, increasing the efficiency of the medical system, and supporting team-based comprehensive care. Requirements

  • Associate degree in early childhood education (ECE) or related major; Bachelor’s degree preferred.
  • Three years of ECE experience.
  • Certified Community Health Worker, or the ability to obtain certification within 24 months of employment.
  • Bilingual Spanish/English required.
  • Possession of a valid CA driver license and insurance. Must be able to travel between sites.
  • Current CPR Certification.
  • Interest in the use of technology to improve care quality a plus - Excel, Word, Outlook, etc.

Responsibilities

  • Adheres to and/or exceeds SPLG’s communication guidelines and expectations with internal and external customers including co-workers, patients, and external partners.
  • Communicates effectively by using welcoming words, proper tone of voice, appropriate body language, eye contact and smiling in all interactions; actively listens and displays a willingness and ability to acknowledge patient and co-workers’ needs, expectations and values using reflective listening and empathy conveyance; responds to co-worker and patient needs in ways that are helpful and beyond expectation.
  • Conducts consultations on a short-term basis for needs regarding development and/or behavioral concerns, including Tier 2 services.
  • Responds to, and tracks call requests within the designated response times, including needed responses for patients assigned to other PPHWs when absent.
  • May participate in ongoing reciprocal feedback meetings with Behavioral Health providers as well as attend various community meetings, as requested.
  • Maintains the community resource directory/database.
  • Provides referrals and tracks follow-up, as appropriate, to help families make successful connections to key resources within the community.
  • May work with the Support Staff department to ensure effective coordination between providers and Pediatric Patient Health Workers, as necessary.
  • May engage in Continuous Quality Assurance and Quality Improvement (CQI) initiatives, as requested.
  • Collaborates in the implementation of the program initiatives including but not limited to universal screenings, providing positive parenting and early learning guidance information available, and recommending any observed workflow changes for optimal efficiency.
  • Works closely with pediatric primary and family practice providers around care coordination, goalsetting, coaching, and education about key aspects of a child’s development.
  • Supports the implementation of shared medical visits and other groups that focus on educating parents about child development and education about other pediatric related topics.
  • Supports the implementation and administration of early education programs, such as Healthy Steps, as applicable, to enhance parent education and facilitate connections to other Salud services. This includes, but is not limited to, any initiatives that contribute to the development and growth of the department.
  • Documents patient encounters in electronic health records (EHR) with appropriate supporting documentation including but not limited to, screening scores and care plan; stage and drop billing codes per the Daily Charge Entry Policy.
  • Supports the accomplishment of the organization's strategic plan and initiatives to achieve high impact and significant improvements in organizational performance.
  • Ensures a productive work environment and achievement of goals through strategies and practices in line with the CARE fundamentals of communication.
  • Completes EHR tasks in a timely manner, noting action taken.
  • Maintains accurate records of individual caseloads using appropriate tracking logs to ensure patient needs are being met, and that referrals and treatment plans are properly followed up and closed.
  • May contact patients due for screening to schedule initial and follow-up appointments when scheduler is absent; schedules any follow-up appointments during patient visits.
