About The Position

This community nursing role involves scheduled visits to care for patients in various settings. As a Registered Nurse (RN) or Registered Practical Nurse (RPN), you will work with patients of all ages with differing complexity of care needs. The role plays a key part in helping patients remain comfortable in their homes for as long as possible. Requirements

  • Registration with the CNO as a Registered Nurse or RPN in good standing
  • Valid driver’s license and access to a vehicle is required
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Passion for making a difference in your community

Responsibilities

  • Using appropriate assessment tools/techniques to assess patients’ individualized needs
  • Executing and continuously monitor care plans
  • Engaging patients using a collaborative approach by building rapport and trust
  • Maintaining and update professional patient records related to patient encounters and plans
  • Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
