The Children’s Health Center and Lenox Baker Children’s Hospital at Duke University Hospital focus on pediatric outpatient clinics, delivering exemplary care for an excellent patient experience. The Pediatric Medicine and Rehabilitation (PMR) Service within Duke Children's provides exemplary care and this position will offer triage support for this patient population, working closely with PMR faculty. The role requires establishing strong rapport with patients and families via phone, coordinating services, and providing education and nursing support both in and out of the clinic setting. The work environment is supportive, dynamic, and challenging, involving both independent work and team collaboration.

Requirements

  • BSN Required
  • 3 years of nursing experience required.
  • Current or compact RN licensure in the state of North Carolina required.
  • Solid background in Pediatric Nursing
  • Experience with telephone triage or managed care coordination
  • Strong oral/written communication skills
  • Software navigation abilities (EPIC experience preferred)
  • Good organizational skills
  • Flexible attitude with the ability to function both independently and as a team member
  • Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others.
  • Knowledge of scope for a registered nurse
  • Knowledge of and appropriate application of the nursing process.
  • Ability to independently seek out resources and work collaboratively.
  • Ability to establish and maintain effective working relationships.
  • Ability to teach patients and families in accordance with the nursing plan of care.

Nice To Haves

  • EPIC experience preferred

Responsibilities

  • Triage calls and resolve problems within scope.
  • Gather relevant information and coordinate follow-up with providers to develop a plan and convey the plan back to patients.
  • Document communication in Maestro care and manage MyChart messages.
  • Assist with medication refills, referral management, and completion of needed forms (DME, school, DMV), letters, etc.
  • Assist in coordinating monthly multidisciplinary clinics.
  • Assist in coordinating diagnostic testing and outpatient appointments.
  • Provide patient and family education on the plan of care for procedures, therapies, and equipment.
  • Partner with social work on patient needs.
