Pediatric In-Home & Acute Care Registered Nurse
About The Position
Imagine Pediatrics is a tech-enabled, pediatrician-led medical group that is reimagining care for children with special healthcare needs. They deliver 24/7 virtual-first and in-home medical, behavioral, and social care, collaborating with families, providers, and health plans to overcome barriers to quality care. This role enhances existing care teams by providing an additional layer of support with compassion and creativity, focusing on children with medical complexity. The role requires at least 80% travel to support episodic in-home care operations across various markets. The schedule is expected to be Monday – Friday 10 am - 6 pm local time with flexibility based on member needs, visit locations, and virtual acute care support. Applicants must reside in Florida, Tennessee, North Carolina, Missouri, Texas, Georgia, Louisiana, or the District of Columbia. An active RN compact licensure is required.
Requirements
- Active RN compact license required.
- PALS & BLS required
- Bachelor's degree in nursing (BSN) is required.
- Minimum 3+ years of pediatric nursing experience, ideally including high-acuity settings (ED, urgent care, transport, or similar).
- 1–2 years in pediatric emergency medicine experience.
- Valid driver's license, reliable transportation, and good driving record.
- Comfort driving long distances in varied weather conditions.
- Strong clinical judgment, communication skills, and ability to work independently in the home environment.
- Comfort with EMRs, virtual platforms, and mobile documentation tools.
Nice To Haves
- EMS (EMT/P) experience and 2+ years of pediatric home care experience preferred
- Bilingual Spanish is preferred
Responsibilities
- Travel to members’ homes and community settings to deliver episodic, longitudinal, and urgent pediatric care.
- Build trusted, compassionate relationships with children and families, ensuring coordinated, culturally responsive care.
- Collaborate within an integrated care team including physicians, APPs, nurses, pharmacists, dietitians, navigators, social workers, and therapists.
- Conduct and document in-home assessments; perform procedures such as ear irrigation, point-of-care testing, IV placement and lab draws, wound care, and medication administration.
- Provide patient and family education, follow-up care, and care coordination using the Imagine Pediatrics platform.
- Flex into the virtual acute care RN role to conduct virtual assessments and manage urgent pediatric needs using established protocols.
- Document care in the EMR (Athena) and communicate with families through Imagine Pediatrics’ proprietary application.
- Utilize virtual platforms and digital tools confidently to support care delivery.
- Perform other duties as assigned.
