About The Position

Angels of Care Pediatric Home Health is seeking compassionate and dedicated Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role focuses on delivering care according to physician orders and active plans of care, ensuring patient well-being and educating family members to promote positive outcomes. The position emphasizes preventative initiatives and maintaining the quality of care within a loving and supportive company culture.

Requirements

• Active RN or LPN/LVN license.
• Ability to provide care in a client home setting.

Responsibilities

• Provide direct, one-on-one medical care to infants, children, and/or adolescents in their homes.
• Focus on the medical needs and treatment of patients according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administer prescribed medication, treatments, and therapies.
• Coordinate care with relevant parties.
• Educate family members on patient clinical care to enhance positive outcomes.
• Implement preventative initiatives to protect the quality of care for patients.
