Requirements

• Current LPN/RN license in the state of Pennsylvania
• Strong understanding of pediatric nursing practices and standards
• Excellent communication and interpersonal skills to build strong relationships with patients, families, and healthcare teams
• Ability to work independently in a fast-paced environment while prioritizing patient safety and satisfaction
• Basic computer skills, including proficiency in electronic medical records and communication systems

Nice To Haves

• Pediatric home care experience is preferred

Responsibilities

• Providing skilled nursing care to pediatric patients in their homes, including wound care, medication management, and IV administration
• Collaborating with patients, families, and healthcare teams to develop and implement individualized care plans
• Conducting comprehensive assessments of patients' physical and emotional needs
• Educating patients and families on self-care techniques, medication management, and disease management
• Documenting patient information and communicating with healthcare teams in a timely and accurate manner
• Working closely with caregivers and home health aides to ensure seamless transitions and continuity of care
