This role involves assessing, planning, implementing, and evaluating total patient care while coordinating patient and family teaching. The Licensed Practical Nurse (LPN) will deliver professional nursing care to assigned patients, assess their physical and emotional status, competency, home environment, safety factors, and family support. The LPN will provide information on patient rights, reimbursement procedures, and patient/family rights. They will evaluate patient status, review and implement care plans, and communicate with attending physicians and clinical management. The role also includes performing nursing procedures and techniques as ordered by the physician, administering medications and intravenous solutions, supervising Home Health Aides, assisting patients with activities of daily living, and maintaining patient clinical records and reports. Ensuring quality care is provided by Home Health Aides and participating in on-call rotation are also key responsibilities. This position requires overseeing patient care, including direction, coordination, and evaluation of care and staff, which involves selection and training of employees, work planning, performance appraisal, and addressing complaints.
