About The Position

Reserve Care (formerly Hospice of the Western Reserve) is seeking a Pediatric Home Care Registered Nurse (RN) or LPN to join their team. This role involves providing palliative nursing care and support to pediatric patients and their families in their homes. Requirements

  • Licensed RN or LPN in the State of Ohio
  • BS or equivalent experience
  • 3-5 Years of experience in Pediatrics, or Hospice with the desire to transfer into Pediatrics
  • Experience in pediatrics and knowledge regarding normal growth and development, pediatric assessment skills, and pharmacology
  • Must work a minimum of 40 hours, four days/week
  • May at times work 4 days, 10-hour shifts based on department needs and supervisor approval
  • 1 evening per week for call coverage and 1 weekend worked per month.
  • Must have valid driver’s license, car available, and insurance coverage as required in Ohio
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
  • Provide an active driver’s license
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics and professionalism.

Nice To Haves

  • Experience preferred in pain and symptom management, oncology, public health, mental health

Responsibilities

  • Assess physical, psychosocial and spiritual needs of the patient and family.
  • Provide intermittent skilled nursing services to the patient under the patient’s physicians’ directions.
  • Monitor and evaluate pain and symptoms and provide information and assistance to the patient and family to manage those problems.
  • Serve as liaison to the patient’s physician(s) when needed; provide information to the physician based on knowledge relating to the patient’s needs.
  • Provide emotional support to the patient and family.
  • Refer and follow up patients and/or family members to other services when appropriate.
  • Provide instruction (health teaching) to the patient and family as related to home care.
  • Maintain and promptly complete all necessary plan and reports related to home care.
  • Participate in care conferences and bereavement meetings.
  • Participate in public relations activities when necessary.
  • Participate in audit review, quality assurance activities and other meetings as assigned.
  • Plan and supervise activities of HHA as related to patient care.
