Requirements

  • Minimum 3 years working with children and adolescents with disabilities, including care coordination or case management.
  • Familiarity with HCBS waivers, pediatric IHSS, and Personal Care programs.
  • Detail-oriented with excellent organizational and time management skills.
  • Confident communicator with the ability to handle escalated issues professionally.
  • Proficient with Microsoft Outlook and EHR systems.
  • Ability to interpret and implement regulatory and procedural changes.
  • Collaborative, accountable, and service-minded.
  • Combination of sitting at a computer and walking/standing as needed for client visits or team supervision.
  • May require lifting up to 20 pounds occasionally.
  • Close visual acuity for reviewing documentation and client records.
  • Requires talking, hearing, and manual dexterity for typing, paperwork, and client interactions.

  • Bilingual in English and Spanish
  • Strong knowledge of Colorado Medicaid regulations for pediatric care and unskilled personal care services

Responsibilities

  • Maintain professional accountability for assigned caseload, ensuring compliance with documentation standards, timelines, and pediatric program requirements.
  • Manage a caseload focused on pediatric clients, including supervisory visits, care plan updates, and service plan meetings as required by program and payer.
  • Collaborate with internal departments (Medical Records, PAR, HR, Intake, Nursing, and Quality) to resolve issues impacting pediatric service delivery.
  • Ensure accurate, timely, and complete documentation is maintained in pediatric client records.
  • Promote consistent, family-centered communication and high-quality service delivery to children, families, and caregivers.
  • Participate in the implementation of workflow improvements, policies, and process changes within the Pediatric Department.
  • Communicate concerns, risks, or barriers to service delivery to Lead Coordinators or management as appropriate.
  • Perform other duties as assigned to support operational and departmental goals.
