About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Pediatric Neurology Clinical Pharmacist to join their team in Nashville, Tennessee. This role involves providing comprehensive pharmacy services to patients in an outpatient neurology setting, working directly with neurology providers and patients. The position focuses on optimizing pharmacotherapy, patient counseling, medication reconciliation, prior authorization review, and assisting with retail prescriptions and outpatient pharmacy navigation. Collaboration with pharmacy, nursing, and medical staff is essential, requiring excellent communication skills. The Clinical Pharmacist will also provide leadership in medication error prevention and adverse drug reaction reporting, contributing to the continuous improvement of medication safety practices within the institution.

Requirements

  • LIC-Pharmacist License - Tennessee
  • Relevant Work Experience
  • 1 year of experience
  • Bachelor's degree

Nice To Haves

  • Completion of ASHP accredited PGY-1 residency program, in a pediatric hospital setting
  • 2 years of pediatric hospital experience

Responsibilities

  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
  • Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
  • Medication reconciliation, optimizing pharmacotherapy, patient counseling, prior authorization review and approval, assisting with retail prescriptions, and working with the patient to navigate outpatient pharmacy services.
  • Collaboration and integration with pharmacy, nursing and medical staff.
  • Leading efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy.
  • Works to continuously improve process and practice in the institution to enhance safety of medication therapy.
