About The Position

NYU Langone Health is seeking a part-time Pediatric Certified Diabetes Care and Education Specialist to join their team. This role involves providing comprehensive diabetes self-management education and training to pediatric patients and their families. The specialist will be an integral part of the interdisciplinary team, supporting the diabetes practice, and contributing to quality initiatives. Responsibilities include patient education, data collection, nutrition therapy, medication management, and facilitating smooth transitions across care settings. The position is based at the Robert I Grossman MD, and Elisabeth J. Cohen, MD, Pediatric Diabetes Center and also serves patients at NYU Langone Orthopedic Hospital/RUSK and Hassenfeld Childrens Hospital at NYU Langone.

Requirements

• Certified Diabetes Educator (CDE) is a health professional (RN/ RD/ NP/ PA).
• Registered or licensed by an appropriate accrediting body in their respective field.
• Minimum of a bachelor's degree; master's preferred.
• Certified by the National Certification Board of Diabetes Educators.
• Basic Life Support for Healthcare Providers certification.
• Background in pediatrics.
• Minimum of one year of diabetes education experience in a hospital setting or outpatient practice.
• Clinical competence in the use of diabetes medications including insulin.
• Clinical competence in the management of all patients with diabetes.
• Clinical competence in all aspects related to education and successful use of continuous subcutaneous insulin pumps and continuous glucose monitoring.
• Clinical competence in the management and modification of individual diabetes plan of care based on optimizing glycemic control and specified targets.
• Ability to effectively communicate with all levels of the organization.

Responsibilities

• Provide comprehensive diabetes self-management education and training to individuals and groups.
• Support the diabetes practice and be an active member of the interdisciplinary team.
• Conduct comprehensive diabetes education for patients, families, and caregivers.
• Collect blood glucose data and monitor outcome measurements.
• Provide nutrition therapy.
• Maintain evidence-based hypoglycemia and hyperglycemia management order sets and protocols.
• Monitor, track, and perform root cause analysis to prevent errors and patient harm.
• Develop individualized medication management plans.
• Create plans of care that facilitate smooth transitions across care settings.
• Participate in quality initiatives and performance development and implementation utilizing evidence-based practices.
• Support physicians in caring for pediatric diabetes center patients with all types of diabetes and pre-diabetes.
• Participate in clinical co-management with physicians and other care providers, providing input into patient education, identifying barriers to care, care coordination and transition, and making appropriate modifications of diabetes medical regimes per protocols.
• Establish policies, protocols, and standards of practice in accordance with best practices.
• Develop and implement the diabetes education curriculum.
• Assist in planning diabetes center events.
• Encourage research to validate practice and improve quality of Diabetes Self Management and Training (DSME/T).
