About The Position

Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity. The primary location for this role is remote, the expected schedule requirement is 4x10s 8:00 am - 6:00 pm (Tuesday-Friday). Active TX licensure is required.

Requirements

  • Minimum 5 years’ experience with a pediatric population required, Community Based Health or Complex Care strongly preferred.
  • You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board certification.
  • Active TX licensure required, and an additional license in MO or IL is preferred
  • Experience and comfort with children with complex medical needs
  • Experience with Medicaid and/or Community health preferred
  • Bilingual Spanish is required.

  • Ability to lead and guide a clinical team.

Responsibilities

  • Manage a panel of complex pediatric patients – Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination.
  • Work in a multidisciplinary team – Work with pediatricians, nurses, pharmacist, dietitian and social workers to provide wrap-around care in a virtual-first model.
  • Empower families – Educate and support parents to navigate their child’s health journey with confidence, ensuring long term health success.
  • Deliver high-quality virtual care – Conduct virtual enrollment visits, acute escalations, sick visits, and post-hospital follow-ups, and even in-home virtual assessments supported by our Mobile Integrated Care team.
  • Ensure continuity of care – Step in when needed to bridge care gaps, coordinating prescriptions, and streamline referrals, to remove barriers to care.
