Requirements

High School Diploma, GED or equivalent.
Louisiana Department of Public Health Certified Nurse Assistant license.
American Heart Association BLS certification.

Responsibilities

Collaborate with multidisciplinary team members to ensure appropriate direct patient care and implementation and documentation of care plan progress.
Maintain and advocate a safe environment for everyone, report to appropriate care team members, and initiate corrective actions.
Monitor and engage with your patients, families, and appropriate care team members concerning significant events, patient condition changes, or patient questions and concerns.
Conduct rounds, answer call lights and ensure adequate assistance with meals, personal and oral hygiene needs.
Participate in the implementation of the plan of care.
Identify problems, concerns, changes in condition and report them immediately to appropriate care team personnel.
Work with nursing staff, physicians, families, consultant personnel and ancillary service providers as directed by nursing supervisor.
Ensure continuity of care, consistent communication, and an exceptional experience for every patient.
Follow established performance standards and duties according to policies and procedures and advise and consult with supervisor when unable to complete assignments or perform tasks promptly, effectively, or safely.
Initiate patient and family education using appropriate resources and continuum of care services.
Contribute to the team by assisting others without being asked.
