About The Position

The primary purpose of this job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves ensuring resident well-being, maintaining a safe and comfortable environment, and accurately documenting care provided. The PCA must adhere to all facility policies and procedures, including those related to resident confidentiality and safety.

Requirements

  • High school education or its equivalent.
  • Proficiency in Microsoft Suite products.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Must occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk, talk, read, or hear.
  • Frequent use of office-related equipment (copier/scanner/fax, telephone, calculator).

Responsibilities

  • Provide routine daily care and services to assigned residents according to their assessment and care plan.
  • Utilize PCC to ensure correct resident information when assisting with ALDS, meals, etc.
  • Report all changes in residents' conditions to the Health and Wellness Director promptly.
  • Record all entries on notes, SPAs, and incident reports in an informative and descriptive manner.
  • Report all observed accidents and incidents to the Executive Director and Health and Wellness Director.
  • Maintain resident confidentiality and report any suspected violations.
  • Report any known or suspected unauthorized attempts to access the community’s information system.
  • Perform only trained care tasks.
  • Ensure resident rooms are ready and comfortable.
  • Greet and escort residents, introduce them to others, and make them comfortable.
  • Inventory, mark, and store residents' personal possessions and clothing.
  • Assist residents with packing and transporting possessions during room changes or discharge.
  • Create and maintain a warm, positive, and calm environment.
  • Participate in shift nursing meetings to identify and correct problem areas.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds and add extra covers as requested.
  • Ensure residents unable to call for help are checked frequently.
  • Answer resident calls promptly and check personal care needs.
  • Assist residents with identifying food arrangements.
  • Keep resident water pitchers clean and filled.
  • Perform after-meal care.
  • Receive nursing reports upon reporting for duty.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures.
  • Assist residents with daily functions (dental/mouth care, bath, hair, dressing/undressing).
  • Keep residents dry by changing clothing and linens when wet or soiled.
  • Assist residents in preparing for medical tests and physical examinations.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Observe and report skin breakdowns to prevent bedsores.
  • Report injuries of unknown source, including skin tears.
  • Observe and record data for disoriented and comatose residents.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse of any resident leaving/missing from the facility.
  • Use only trained equipment and report defective equipment.
  • Maintain confidentiality of resident care information and report unauthorized disclosures.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident refusal of treatment requests and report them to supervisors.
  • Give back rubs as instructed.
  • Assist residents with hair care, nail care, and shaving.
  • Position bedrest residents correctly and comfortably.
  • Assist residents with bowel and bladder functions.
  • Maintain intake and output records.
  • Keep incontinent residents clean and dry.
  • Check and report bowel movements and stool character.
  • Utilize protective and adaptive equipment.
  • Provide eye and ear care.
  • Weigh and measure residents as instructed.
  • Measure and record temperatures, pulse, and respirations (TPRs).
  • Assist with the care of end-of-life residents and post-mortem care.
