About The Position

The primary purpose of this job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves ensuring resident well-being, maintaining a safe and comfortable environment, and accurately documenting care provided. The PCA will work closely with nursing staff and other facility personnel to meet resident needs.

Requirements

  • High school education or its equivalent.
  • Proficiency in Microsoft Suite products.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Must occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk, talk, read, or hear.
  • Frequent use of office-related equipment including copier/scanner/fax, telephone, and calculator.

Responsibilities

  • Provide routine daily care and services to assigned residents according to their assessment and care plan.
  • Utilize PCC to ensure correct resident information when assisting with ALDS, meals, etc.
  • Report all changes in residents' conditions to the Health and Wellness Director promptly.
  • Record all entries on notes, SPAs, and incident reports in an informative and descriptive manner.
  • Report all observed accidents and incidents to the Executive Director and Health and Wellness Director.
  • Maintain resident confidentiality and report any suspected violations of protected health information disclosure.
  • Report any known or suspected unauthorized attempts to access the community's information system.
  • Perform only trained care tasks.
  • Ensure resident rooms are ready for occupancy, including making beds.
  • Greet residents and escort them to their rooms.
  • Introduce residents to roommates, other residents, and personnel.
  • Make residents comfortable.
  • Inventory, mark, and store residents' personal possessions and clothing.
  • Assist residents with packing personal possessions for transfers or discharges.
  • Transport residents to new rooms or receiving areas.
  • Assist with loading/unloading residents to/from vehicles.
  • Create and maintain a warm, personal, and positive atmosphere, ensuring a calm environment.
  • Meet with nursing personnel to identify and correct problem areas and improve services.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make and adjust beds as requested.
  • Ensure residents unable to call for help are checked frequently.
  • Answer resident calls promptly.
  • Check residents routinely to ensure personal care needs are met.
  • Assist residents with identifying food arrangements, especially those with visual impairments.
  • Keep residents' water pitchers clean and filled, within easy reach.
  • Perform after-meal care, including removing trays and cleaning residents' hands, face, and clothing.
  • Receive nursing reports upon reporting for duty.
  • Perform all assigned tasks in accordance with established policies, procedures, and supervisor instructions.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental and facility personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions such as dental and mouth care, bathing, hair combing, dressing, and undressing.
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents in preparing for medical tests.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing residents for physical examinations.
  • Observe and report skin breakdowns to prevent pressure ulcers.
  • Report injuries of unknown source, including skin tears.
  • Observe and record data for disoriented and comatose residents.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse of any resident leaving or missing from the facility.
  • Use only trained equipment.
  • Report defective equipment to the Nurse Supervisor/Charge Nurse.
  • Maintain confidentiality of all resident care information.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident refusal of treatment requests and report them to a supervisor.
  • Give back rubs as instructed.
  • Assist residents with hair care functions.
  • Assist residents with nail care.
  • Shave male residents.
  • Keep hair on female residents clean as instructed.
  • Position bedrest residents correctly and comfortably.
  • Assist residents with bowel and bladder functions.
  • Maintain intake and output records.
  • Keep incontinent residents clean and dry.
  • Check and report bowel movements and character of stool.
  • Utilize protective and adaptive equipment.
  • Provide eye and ear care.
  • Weigh and measure residents as instructed.
  • Measure and record temperatures, pulse, and respirations (TPRs).
  • Assist with the care of end-of-life residents.
  • Assist with post-mortem care as instructed.
