About The Position

The Patient Transfer Coordinator Nurse facilitates patient throughput between all UTMB campuses and non UTMB hospitals ensuring appropriate bed placement and MD to MD recorded conference while adhering to EMTALA regulations and hospital guidelines.

Requirements

  • Associate’s degree in Nursing
  • Three (3) years of inpatient experience in a hospital environment
  • Valid State of Texas Professional Nursing (RN) License
  • BLS

Responsibilities

  • Provide guidance to both internal and external physicians by utilizing clinical skills and knowledge.
  • Ensure all internal policy and procedures and federal law are followed by all departmental staff regarding hospital transfers including reviewing transfer cases for compliance.
  • Work closely with the Clinical Operations Administrator (COA) to determine capacity when needed in order to provide our physicians complete information prior to coordination of the transfer call with referring physicians.
  • Provide clinical education and support to the Patient Placement Coordinators as needed to facilitate appropriate patient placement.
  • Use clinical skills and knowledge, complete quality review of all transfers and resulting entry into EPIC, TeleTracking, and other Database systems to ensure they are complete and accurate.
  • Facilitate productivity, team building, and high team morale in the department.
  • Consistently communicate and display oneself in a professional and courteous manner.
  • Practice and promote the principles of patient confidentiality, including adhering to HIPAA guidelines.
  • Provide Manager or Designee updates regarding placement needs or other issues related to patient transfer referrals.
  • Provide back up to Patient Placement Coordinator job function as needed.
  • Serves as a direct contact with the COA and hospital executive leadership regarding bed placement for interfacility and inter campus transfer issues during the shift.
  • Maintain licensure, adheres to UTMB’s standards of conduct/mission and vision, professional code of ethics and standards of practice.
  • Participates in the team meetings and activities, departmental trainings, problem solving, and new process planning and to support the philosophy and goals of the team, department, and institution.
