The Center of Population Health Accountability (CPHA) is founded on a bold strategic vision to provide integrated “whole-person” physical and behavioral health services, including partnerships to address the social determinants of health (poverty, housing, transportation, food insecurity, etc.) that affect a person’s health and well-being. Through a network of community health partnerships, the CPHA emphasizes early intervention and preventive care, both in conventional health care setting and non-traditional community locations, such as neighborhood pharmacies, barber and beauty shops, schools, outreach events, mobile units and telehealth. The Patient Service Representative (PSR) on the Mobile Health Unit entails providing support for the health screenings and additional medical services provided via the units.
