About The Position

The primary function of the Patient Services Coordinator (PSC) is to schedule patient services and coordinate with staff. The PSC is also expected to cross-train as the Medical Records Specialist (MRS). This role involves completing workflow tasks in the Agency’s electronic medical record, prioritizing the scheduling of new patient admissions, and coordinating care with nurses, patients, and other interdisciplinary team members. The PSC provides effective communication to patients, staff, other healthcare professionals, and referral sources, responding to Agency and patient needs professionally and creatively. The role also requires demonstrating commitment, professional growth, and competency, participating in Agency-sponsored in-service training and the Agency’s QAPI program. If clinical, the PSC will perform patient visits and complete documentation timely, while promoting the Agency philosophy and administrative policies to ensure quality of care. Cross-training as the Medical Records Specialist (MRS) is also a requirement.

Requirements

  • High School graduate.
  • Current driver’s license.
  • Reliable transportation and valid and current auto liability insurance.
  • If required to make patient visits, a current CPR card must be required.

Nice To Haves

  • Graduate of an accredited school of vocational nursing, preferred.
  • If clinical, must have a current state license as a Licensed Vocational Nurse or Licensed Practical Nurse.
  • A multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) is preferred.
  • One year of experience in homecare (e.g., home health or hospice) is preferred.
  • If an LVN, nursing skills as defined as generally accepted standards of practice.
  • Good interpersonal skills.

Responsibilities

  • Complete workflow tasks and other assignments to the PSC Responsible Position in the Agency’s electronic medical record.
  • Prioritizes the scheduling of new patient admissions in coordination with the Admin or CM-- RN.
  • Participates in coordinating care with nurses, patients, and other interdisciplinary team members.
  • Provides effective communication to patients, staff members, other health care professionals and referral sources.
  • Respond to Agency and patient needs professionally and creatively.
  • Demonstrates commitment, professional growth, and competency.
  • Participates in Agency-sponsored in-service training.
  • Participates in the Agency’s QAPI program, as directed.
  • If clinical, perform patient visits as assigned and complete documentation timely.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.
  • Cross-trains as the Medical Records Specialist (MRS) as needed.
