The Patient Services Coordinator, LPN/LVN acts as a vital link between clinical staff and patients, along with their families and caregivers. Responsible for scheduling, organizing and managing all aspects of a patient's healthcare maintenance and treatment. Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation. The Patient Services Coordinator-LPN is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. The role also assists with day-to-day office and staff management.
