The Patient Safety Specialist serves as a patient safety and high reliability resource to staff, leaders, and physicians. This role acts as a subject matter expert in general patient safety topics, causal analysis, and action planning. The specialist collaborates with leaders, medical staff, and others to achieve patient safety goals aimed at reducing harm. They facilitate team-based causal analysis using evidence-based methodology, and develop and assist with the implementation of action plans and related training. The role also involves assisting with agenda planning and data preparation for various committees, creating and disseminating safety lessons-learned information, and providing support for integrating patient safety practices into daily operations. Additionally, the specialist acts as a visible patient safety champion for frontline staff, completes chart reviews for harm events, and provides training on safety topics. They coordinate culture of safety surveys, action planning, and goal setting, and prepare reports of survey data. The role also assists with the patient safety practices section of the Hospital Leapfrog Survey. In clinical risk management, the specialist maintains the hospital-wide incident reporting system, tracks and trends reported incidents, and prepares aggregate data for committees. They ensure consistent classification, assign investigations, and ensure timely closure of incidents. The specialist investigates equipment-related problems, assists with medical error disclosure, and makes mandated state reports regarding adverse events. They maintain and update the hospital’s annual Risk Management Plan and report, and assist with risk management policies. The role performs hospital risk assessments, FMEAs, and conducts risk investigations. They provide training to staff and providers to enhance awareness of their role in reducing liability exposures and maintain awareness of legislative and regulatory activities related to healthcare risk management. Information on claim patterns, risk control, and regulatory changes is disseminated as needed. The specialist collaborates with the Compliance Department and legal counsel, and works with legal counsel to coordinate the investigation, processing, and defense of claims against the hospital. They collect and provide legal counsel with necessary information and documents for testimony preparation. In general duties, the specialist assists with survey readiness and management activities, leads and/or assists multidisciplinary performance improvement teams using established problem-solving methodologies, and performs project-based chart reviews and staff education. They effectively manage assigned projects with independence, act as the department representative on multidisciplinary teams and committees, support clinical operations and initiatives, and attend required hospital-wide orientations, meetings, and in-services. Professional growth and development are maintained through seminars, workshops, and professional affiliations. Other duties as assigned include coverage of essential department functions and services.
