About The Position

The Patient Safety Quality Consultant acts as a change agent with deep knowledge of complex clinical processes, clinical information systems, patient safety, project management, clinical operations, data analytics and design of clinical processes that deliver highly reliable patient care. The position supports all aspects of the Patient Safety program for University of Utah Health, including work to identify, investigate, analyze, and address factors that cause harm, pose risks to patients or which contribute to patient harm. Responsibilities also include working across the organization with physician and operational leaders, Nursing Quality Directors, Allied Health, the Departments of Risk Management and Quality, Patient Experience, Resiliency Center, Graduate Medical Education, faculty and direct service staff throughout acute care, primary care, outpatient services and academic service lines. The intent of this position is to increase the capability of the organization to provide safe care, enhance the adoption of safe clinical practices, work to embed the culture of safety throughout the organization and to develop a culture of continual learning. This position has no responsibility for providing direct care to patients.   • Bachelor's degree in a health care clinical or business related field or equivalency.
  • One year clinical or healthcare experience.

Nice To Haves

  • Licensed or accredited health care professional (Registered Nurse, Physical Therapist, Occupational Therapist, Pharmacist) or relevant clinical experience
  • Lean Six Sigma Greenbelt (LSSGB), Project Management Professional (PMP), Certified Professional in Healthcare Quality (CPHQ), Performance Improvement Facilitation, Human Factors, or equivalency.
  • Minimum two (2) years' experience leading/facilitation process improvement projects in a healthcare setting.
  • Minimum two (2) years of experience in health care, performance improvement, quality improvement and/or patient safety.
  • Master’s Degree in a healthcare focus or equivalent experience (One year of education is equivalent to two years of experience).
  • Certified Professional in Patient Safety (CPPS) from IHI. Must be completed within 12 months of hire.
  • Minimum two (2) experience in electronic medical records.

Responsibilities

  • Assists the Chief Quality Officer and Patient Safety Director as necessary, in preparation of special requests for information regarding Patient Safety events and activities
  • Reviews, triages, and analyzes patient safety events and related data to identify trends, assess contributing factors, ensure data integrity, and translate findings into actionable improvement opportunities.
  • Serves as a back-up Patient Safety event reporting system administrator which includes system problem-solving, database management, data querying, report-writing and delivery.
  • Analyzes and synthesizes patient safety data to develop reports, dashboards, and presentations for leadership, committees, and operational teams, including ad hoc information requests.
  • Leads and collaborates on patient safety and quality improvement initiatives, including evaluating current processes, developing standard work, and implementing evidence-based strategies to reduce harm.
  • Leads causal analysis for serious safety events and oversees accountability of corrective action plans across all levels of the organization
  • Report events to the state per Utah State requirements
  • Manages the protections of Patient Safety Work Product (PSWP) in assuring that data and information is exchanged according to the Patient Safety Organization and Patient Safety and Quality Improvement Act.
  • Provides education, coaching, and mentorship to staff, leaders, and new team members on patient safety principles, reporting processes, and high-reliability practices.
  • Serves as an organizational leader and subject matter expert in patient safety, representing the function in committees and providing consultation for accreditation and regulatory readiness.
