About The Position

Under the direction of the Patient Safety Officer, the Patient Safety Coordinator promotes and supports clinical quality and the Adena Health System's patient safety program. This role assists with process improvement, which supports the reduction of medical/health errors, and other factors that lead to unintended adverse patient outcomes.  Core responsibilities include assistance with safety assessments, activities of the Patient Safety Committee, the adverse event reporting system, cause analysis, follow-up, and provides education on the system-based causes of medical errors.

Requirements

  • Bachelor's Degree in Healthcare Related Field
  • Active Ohio RN Licensure or clinical licensure in area of expertise.
  • Certified Professional in Patient Safety CPPS -Upon hire or within 2 years of hire.
  • Certified Professional in Healthcare Quality -CPHQ within 5 years of hire.
  • Minimum 5-7 years of experience in clinical healthcare.

  • 3 years of experience in a Patient Safety Program.

Responsibilities

  • Investigation and analysis of adverse event reports. Provides general support for and monitors/maintains required participation in the High Reliability Platform event reporting system. Classifies each event utilizing the PSO Safety event classification levels of harm document.
  • Assists the Patient Safety Director in planning activities to ensure dissemination of information for hospital wide improvement efforts.
  • Provides support for HRP events platform by managing data processes including event reporting deadlines, monitoring data accuracy and data verification.
  • Coordinates and facilitates the work of leaders assigned to cause analysis events/teams including data completion, cause analysis and improvement activities. Leads and facilitates RCA's across the health system. Facilitates Root Cause Analysis (RCA) activities, in conjunction with executive leaders, leaders and staff. Facilitates Apparent Cause Analysis (ACA) activities. Provides education and support to all areas of the health system to promote patient safety and high reliability principals. Reviews policies & procedures as assigned aimed at reducing medical errors and patient harm. All other patient safety duties as assigned by the Director of Patient Safety. Tracking and oversight of all performance improvement plans including ACA and RCA events. Ensure actions are implemented and reported to cause analysis team.
  • Assists in preparation of patient safety reports and documents for the patient safety and quality committees. Abstracts submits/reports data for publicly reported patient safety indicators and external benchmarking programs. Stays abreast of current patient guidelines, regulations and recommendations.
  • Facilitates team activities to communicate event/classification definition changes, data collection issues, data interpretation issues, event reporting updates. Identifies best practices needed to support patient safety initiatives and integrates into patient safety team practices.
  • Performs adverse event data review for presentation at the weekly Safety Event Review Team -SERT Meeting. Schedules and facilitates SERT meeting with leaders across the health system.
  • Reviews & Inputs data received into the great catch program data base and coordinates/assists leaders in the presentation of awards to staff.
  • Provides project management & data gathering/analysis and decision making support for department initiatives focused on improvements in patient safety with focus on the National Patient Safety Goals and other initiatives related to patient safety.
  • Provides education and support to all areas of the health system to promote patient safety and high reliability principals. Reviews policies & procedures as assigned aimed at reducing medical errors and patient harm. All other patient safety duties as assigned by the Director of Patient Safety.
