The Patient Relations Coordinator will coordinate all aspects of the UH Select Program by effectively and efficiently handling member and physician needs. This role involves assessing and evaluating patients' care needs, serving as a role model and resource, and providing medical assistant coverage for Cleveland-based practices. The coordinator will manage the day-to-day activities of UH Select Cleveland practices, including database management, supply ordering, test result retrieval, medication refills, EMR maintenance, scheduling, and other system responsibilities. They will also provide daytime call coverage for the program access line, assist staff, and coordinate with other hospital departments to ensure excellent patient experiences and optimal care outcomes. The position assumes a leadership role in the absence of the Supervisor, taking responsibility for the organization, coordination, and administration of clinical practice and patient care delivery within UH Select, focusing on standards of care, operational excellence, service excellence, and quality outcomes. Staffing responsibility for UH Select practices, creating orientation and onboarding processes, and developing new and annual training for employees are also key functions. The role involves building strong partnerships with organizational leaders to achieve practice goals, ensuring clinical practice standards are met in accordance with regulations and accrediting bodies, and developing/implementing quality improvement and employee engagement programs. Participation in professional development activities and the growth of the clinical team, along with incorporating acceptable standards of professional practice to improve patient safety, quality, and satisfaction, are essential. Additional responsibilities include performing other duties as assigned, complying with all policies and standards, and adhering to all requirements for safely and securely maintaining Protected Health Information (PHI).
