About The Position

Assists with the daily functions, patient outreach, and identification of community resources for patients, patient counseling and education for the multi-disciplinary clinic. Duties: Responsible for patient counseling and referrals for patient needs. Assists with coordinating patient care. Responsible for the on-going daily management of patient continuity of care for patients entering and going through the multi-disciplinary clinics. Organizes and assists with conducting community outreach on behalf of the Program. Maintains a customer service program to ensure a high quality service and customer satisfaction for patients in the clinical setting. Creates and conducts surveys for evaluating satisfaction of the users of the program. Participates in and initiates advancement of hospital, community and patient relations. Survey and follow-sup with referring providers to ensure high quality customer service. Meets with patients to ensure customer satisfaction with the Program and hospital experience. Utilizes and/or creates databases for tracking and following patients through out-patient clinical settings. Organizes and analyzes program data on a regular basis. Creates and maintains referring physician informational databases. Maintains all documentation and files on behalf of the program. Performs various duties as needed to successfully fulfill the function of the position.

Requirements

  • Bachelor's Degree in Health Education, Social Work, Psychology, Nursing, or health related field AND:
  • 12 months experience in a medical or clinical care setting.
  • Equivalency/Substitution: Will accept 48 months related experience in lieu of the Bachelor's Degree for a total of 60 months experience.
  • A license from the State of Oklahoma related to the required health field.
