Capital Imaging - A Rezolut Partner is looking for a Patient Navigator. Requirements

  • High School degree or equivalent
  • Minimum 5 years healthcare experience required
  • Previous imaging experience a plus
  • Self-directed learner who is willing to apply direct feedback and continuously and humbly self-assess in order to grow personally and professionally.
  • Ability to work under general guidance with some expectation of autonomy.
  • Excellent verbal and written communication skills.
  • Strong attention to detail.
  • Ability to prioritize tasks and to delegate them when appropriate.
  • Acts with integrity, professionalism, and confidentiality.
  • Demonstrates a working knowledge of employed information systems.

Responsibilities

  • Coordinate continuity of care for breast imaging patients.
  • Schedule appointments by interfacing with referring physicians’ offices, patients, and patients’ family members.
  • Monitor patient flow.
  • Assist technologists.
  • Organize compliance documentation.
  • Assist visitors with their needs.
  • Provide excellent customer service.
  • Greet customers in a polite and prompt manner.
  • Kindly and patiently assist clients by answering their questions and helping with their requests.
  • Ensure necessary information/scripts/paperwork are in order prior to patients’ arrival.
  • Review or complete exam questionnaires with patient as required.
  • Scan appropriate information to patient chart in RIS.
  • Create detailed notes in patient chart in RIS documenting pertinent information.
  • Assist technologist and Radiologist during biopsy procedures.
  • Work callback list and schedule appropriately.
  • Confirm appointments and follow-up with patients regarding missed appointments.
  • Assure Radiologist receives pathology reports and contacts patient if provider requests.
  • Schedule surgical consultations as needed, send necessary reports and images to surgeon.
  • Follow up with providers to ensure receipt of positive findings and all biopsy reports.
  • Organize and mail patient letters.
  • Assist with MammoSave texts.
  • Troubleshoot patient portal issues.
  • Deliver patients to appropriate modality when needed.
  • Communicate with technologists to keep patient flow moving.
  • Communicate schedule delays with patients and their families.
  • Maintain patient confidentiality.
  • Send images/reports to requesting facilities.
  • Assist technologists in compiling necessary documentation for MQSA and ACR.
  • Demonstrate a working knowledge of employed information systems.
  • Demonstrate enthusiastic commitment to providing excellent customer service.
  • Maintain telephone call list to notify patients of openings or changes in appointment times and schedule accordingly.
  • Maintain work area in a neat and orderly manner.
  • Perform other related duties as directed.
