About The Position

As a Patient Navigator, you will play a vital role in helping patients successfully navigate the healthcare system. You will serve as a trusted resource and advocate, assisting patients with referrals, prior authorizations, appointment coordination, and access to community resources. This position requires strong communication skills, attention to detail, and a commitment to ensuring every patient receives the support they need throughout their healthcare journey.

Requirements

  • High school diploma or equivalent required.
  • Strong computer proficiency and ability to learn healthcare software and electronic medical record (EMR) systems.
  • Excellent verbal and written communication skills.
  • Ability to work effectively in a fast-paced, high-volume environment while maintaining professionalism and attention to detail.
  • Demonstrated ability to manage multiple priorities and maintain composure under pressure.
  • Tdap (current booster)
  • MMR (Measles, Mumps, and Rubella) vaccination series
  • Varicella (Chickenpox) vaccination
  • Hepatitis B vaccination series
  • Annual Influenza (Flu) vaccination
  • Tuberculosis (TB) screening

Nice To Haves

  • Knowledge of medical terminology.
  • Three to six months of experience in a healthcare setting or related industry.
  • Experience with patient referrals, care coordination, or healthcare navigation.
  • Understanding of health insurance processes and prior authorization requirements.
  • Familiarity with Medicaid and insurance verification procedures.

Responsibilities

  • Coordinate referrals and specialty appointments based on provider recommendations and patient needs.
  • Educate patients regarding referrals, appointments, procedures, and follow-up care.
  • Ensure referral completion and facilitate timely communication between providers, specialists, and patients.
  • Assist patients in understanding and accessing available healthcare services.
  • Help patients overcome barriers to care by connecting them with community resources and support services.
  • Assist patients with access to financial assistance programs, social services, legal resources, specialty care, and other community-based programs.
  • Serve as a resource for patients navigating complex healthcare systems and processes.
  • Maintain accurate tracking of referrals and authorization requests.
  • Monitor pending referrals and authorizations to support timely patient care.
  • Maintain accurate patient records within the electronic medical record (EMR) system.
  • Track patient outcomes, referrals, recalls, and follow-up activities.
  • Prepare reports and statistical information as required.
  • Manage patient recall lists and assist with preventative care outreach efforts.
  • Work closely with providers, nurses, medical assistants, and administrative staff to ensure seamless patient care.
  • Participate in clinic meetings, quality improvement initiatives, and organizational projects.
  • Support grant-funded programs, patient outreach initiatives, and community health efforts.
