The Patient Navigator plays a critical role in guiding patients through their healthcare journey and improving access to care. Responsibilities include assisting patients with check-in and check-out processes, scheduling appointments, verifying insurance eligibility, supporting applications for insurance and sliding fee programs, and connecting patients with community resources. The Patient Navigator serves as a key point of contact for patients, helping them navigate healthcare services, reduce barriers to care, and ensure a positive patient experience. This position actively participates in quality improvement initiatives and supports patient-centered care efforts.

Requirements

  • High School Diploma or equivalent.
  • Proficiency with Microsoft Office applications and basic computer skills.
  • Fluency in English and Spanish.
  • Active BLS certification
  • A valid driver’s license.
  • Reliable transportation.

Nice To Haves

  • Community Health Worker (CHW) Certification.
  • Previous experience in healthcare, patient services, customer service, or an administrative role.
  • Previous cash handling, payment processing, or financial transaction experience.
  • Experience working with underserved or vulnerable populations.
  • Familiarity with electronic medical records (EMR) systems and insurance portals.

Responsibilities

  • Guide patients through various processes, including check-in, intake, and check-out procedures.
  • Schedule and coordinate follow-up appointments, ensuring timely and accurate bookings.
  • Remind patients of upcoming appointments via phone calls.
  • Handle phone calls to and from patients, addressing inquiries and providing information as needed.
  • Communicate referral information to patients, ensuring they understand the next steps in their care.
  • Assist patients with understanding their insurance benefits and coverage.
  • Connect patients with community resources and support services.
  • Conduct outreach and follow-up calls to improve patient engagement and continuity of care.
  • Assist patients with overcoming barriers to healthcare access, including transportation, insurance, and resource-related challenges.
  • Process patient payments, maintain accurate cash handling records, and assist patients with billing, copays, sliding fee scale discounts, and payment-related inquiries.
  • Participates in community outreach events, health fairs, and engagement activities to increase awareness of available services.
  • Assists with identifying and engaging individuals who may benefit from healthcare and social support services.
  • Verify patient identification to ensure accurate and secure records.
  • Assist patients with completing intake paperwork and applications, including the sliding fee scale program application.
  • Educate patients on the importance and completion of required documents.
  • Verify patient insurance eligibility and navigate insurance portals to obtain necessary information.
  • Maintains patient confidentiality and complies with all HIPAA, HRSA, and organizational privacy and security requirements.
  • Maintains a courteous and professional demeanor; greets individuals and patients as they enter and exit the clinic; facilitates appropriate communication and interpersonal skills with all staff members, volunteers, and patients.
  • Maintains safe, secure, and healthy work environment by establishing and following standards and procedures; complies with legal regulations.
  • Ensures front office and clinic policies and procedures (as applicable) are followed at all times.
  • Meets established productivity, documentation, and patient engagement goals.
  • Performs other duties as assigned.
