The Patient Navigator facilitates patients' ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers. This role involves conducting patient evaluations, ensuring access to insurance and social service programs, acting as a liaison between patients and providers, and educating patients on health-related issues. The Patient Navigator must maintain current knowledge of community resources and adhere to University policies.
