About The Position

Brentwood Hospital is seeking a dedicated Patient Navigator to join their team, focusing on improving mental health outcomes. This role involves managing the step-down process for patients transitioning from inpatient care to outpatient services, ensuring they are informed about and engaged with available options like PHP/IOP/ECT prior to discharge. The navigator will also be responsible for data collection, communication with departments and the CEO, and reporting for process improvement. Brentwood Hospital is a 260-bed psychiatric and chemical dependency treatment facility with over 45 years of service, providing quality care to a diverse patient population in a safe and nurturing environment.

Requirements

Associates degree in nursing from an accredited program of professional nursing or Master’s degree in behavioral health related field required.
Previous mental health experience preferred.
Must be competent and comfortable in dealing with all aspects of the mental health field.
RN or LPN, preferred

Responsibilities

Manages the workflow of the step-down process by ensuring eligible patients are identified and engaged in PHP/IOP/ECT discussion as a next step in treatment prior to inpatient discharge.
Meets with every eligible patient to discuss PHP/IOP/ECT or outpatient services, methods of transportation, and to identify any obstacles that need to be resolved before discharge. This engagement will begin within 24 hours upon admission to the hospital.
Submits data to the PI committee and periodically reviews step-down navigation for process improvement.
Independently plan and organize one's own schedule and activities.
