About The Position

The Tidelands Health's Cancer Center Patient Navigator (PN) aims to guide newly diagnosed cancer patients from surrounding communities to receive timely, patient-centered care at the TH Cancer Center Network. The PN will build trusting relationships with patients, facilitate their appointments at the TH Cancer Center, and maintain close communication with the patient's entire care team, adhering to defined goals and identified patient needs. The PN will assist patients in coordinating and completing appointments within the TH Cancer Center, working to reduce barriers to timely cancer care and treatment, and improving access to high-quality cancer care for underserved populations. Recognizing the critical importance of addressing supportive care needs from the outset of a cancer diagnosis, the PN will administer surveys to assess patients' symptom burden, illness perception, financial distress, and self-efficacy, which will guide future navigation and care coordination efforts. Interventions may include education, appointment reminders, assistance with insurance and transportation, guidance within the Cancer Center, help with childcare, interpretation services, referrals to psychosocial and/or palliative care teams, and directing patients to appointments. The PN will integrate screening for severe mental illness and risk factors for care disruptions at diagnosis, monitor distress, and streamline referrals to social work, psychiatry, and palliative care. The PN will serve as a liaison between the patient, oncology, and primary care teams, ensuring open communication. The PN will also participate in other related program activities as needed.
