About The Position

This role is part of the Ascension St. John Bedrock Initiative, focusing on leading holistic health and wellness initiatives for the faith-based community in North and Northwest Tulsa. The Patient Navigator will be the primary full-time heart and vascular clinic-embedded navigator, collaborating with community health workers and supporting program integration. The position involves building trusted relationships with congregations and local partners to promote health equity and connect individuals to essential resources. Responsibilities include providing health screenings, culturally appropriate education, health coaching, and care navigation in both community and clinical settings. The role serves diverse populations and denominations, respecting cultural and gender norms, and aims to strengthen community well-being through collaboration and program development.

Requirements

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required

Nice To Haves

  • Proficient computer skills, to include Google Workspace
  • CHW-C or at least 2 years of experience as a Community Health Worker (CHW), Health Navigator or Promotora
  • Familiarity with community organizations and resources available to patients
  • Available to work most Sundays and routine evenings for meetings and community events in support of the Bedrock Initiative

Responsibilities

  • Lead holistic health and wellness initiatives for the faith-based community in North and Northwest Tulsa as part of the Ascension St. John Bedrock Initiative.
  • Coordinate as the primary full-time heart and vascular clinic-embedded Patient Navigator, collaborating with community health workers and supporting program integration.
  • Build trusted relationships with congregations and local partners to promote health equity and connect individuals to essential resources.
  • Provide health screenings, culturally appropriate education, health coaching, and care navigation in both community and clinical settings.
  • Serve diverse populations and denominations, respecting cultural and gender norms, while strengthening community well-being through collaboration and program development.
