About The Position

The Patient Navigation position offers individualized assistance to patients, families and caregivers to overcome healthcare barriers. Patient Navigators work in a variety of settings—community, hospital, home, primary care, and tertiary care. It is their top priority to make their patients feel comfortable and guided throughout their journey. Requirements

  • Bachelor's degree in healthcare related field preferred
  • 2+ years of experience working in a medical setting with oncology patients and families.
  • Ability to prioritize to work and work independently
  • Ability to work independently.
  • Ability to handle stress and high workload volumes.
  • Ability to communicate denial of services to members and providers.
  • Ability to lift up to 35 pounds without assistance.

Nice To Haves

  • Healthcare and oncology experience a plus

Responsibilities

  • Coordinate appointments with providers for a timely delivery of diagnostic and treatment services.
  • Monitor patient satisfaction with their cancer experience and works with team(s) to address any identified issues or barriers.
  • Advocates on behalf of the patient and empower the patient to self-advocate.
  • Ensures that appropriate medical records are available at scheduled appointments.
  • Arranges transportation assistance with available resources.
