About The Position

The Patient Navigator – Licensed Vocational Nurse (LVN) coordinates access of patients through the healthcare system from initial medical diagnosis or point of referral. Gathers all historical medical and insurance information for physician review during initial patient consultation. Functions as a liaison between patients, families, outpatient oncology clinics, primary care physicians, and other support services. Is assigned to and performs patient-oriented procedures and tasks within the LVN scope of practice, education and training. Also performs wound care functions and manages durable medical equipment (DME) supplies. Performs Patient Navigator services within assigned departments. Performs other duties as needed.

Requirements

  • High School Diploma or GED required.
  • Graduate from a school for Licensed Vocational Nurse (LVN) required.
  • Minimum one year experience in a medical front office setting and/or a hospital based call center that schedules appointments for multiple clinics or departments required.
  • Active California Licensed Vocational Nurse (LVN) license required.
  • Basic Life Support (BLS) certification from the American Heart Association (AHA) required.
  • Knowledge of medical terminology.
  • Able to read; write legibly; speak in English with professional quality.
  • Able to use computer and software programs necessary to the position.
  • Able to troubleshoot and calibrate patient care equipment.
  • Able to relate and communicate positively, effectively, and professionally with others.
  • Able to be assertive and consistent in following and/or enforcing policies.
  • Able to work calmly and respond courteously when under pressure.
  • Able to lead, supervise, teach, and collaborate.
  • Able to accept direction.
  • Able to communicate effectively in English in person, in writing, and on the telephone.

Nice To Haves

  • Minimum one year of experience working with Oncology patients preferred.
  • Spanish preferred.
