About The Position

The Patient Navigator – Enhance Care Management (ECM) Program is a community-based professional who serves as a guide to help individuals access and navigate the health care system, as well as related social and financial services, to improve health outcomes. The Patient Navigator performs a variety of health education, clerical, and medical support tasks both in the clinic and in the community. A primary responsibility of this role is to establish and maintain communication between patients, their families, providers, and the health care system to support coordinated care.

Requirements

  • High school diploma or GED.
  • Bachelor’s degree in a related field, or at least three years of progressive experience.
  • Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy.
  • Good written and verbal communication skills.
  • Ability to be trained to perform a variety of tasks.
  • Direct knowledge of the local community.
  • Strong interest in the community and its people
  • Committed to ongoing learning and the ability to model the behavior and activities that support individual development and community capacity-building.
  • Effective communication and other interpersonal skills that can ensure an ability to work with local people and organizations.
  • Ability to effectively assist patients across various age groups, including infants, adults, and individuals with disabilities.
  • Fluency in English (speaks, read and writes).
  • Fluency in Spanish (speaks, interpret, read, and write) and will be required to complete the mandatory Medical Interpretation course offered at NEVHC.
  • Ability to maintain absolute confidentiality about health care and other patient/client information.
  • Ability to maintain a professional demeanor at all times.
  • If working in a school based site, must be willing to submit fingerprints in a manner authorized by the California Department of Justice, with no adverse findings.
  • Current California driver’s license, appropriate insurance coverage, and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
