POSITION SUMMARY: The Patient Navigator is a patient-centered service delivery position focused on improving health outcomes, increasing provider-patient connectedness, and eliminating barriers to care for Transgender, Gender Non-Conforming, and Non-Binary (TGNCNB) individuals across PPNCNY’s service area. The Patient Navigator serves as the primary connector between TGNCNB patients and affirming behavioral health and community supports available through PPNCNY’s partnership with Prism Counseling & Advocacy. This position focuses on facilitating access to telehealth mental health services, harm reduction supports, and LGBTQ+ community groups, while also supporting centralized follow-up and coordination of gender affirming services within PPNCNY. The position must exemplify the commitment to PPNCNY’s core values of teamwork, compassion, confidentiality, quality of care, and mission of the organization. This position is full-time, 40 hours per week and is a hybrid position.
