In this role, the Patient Navigation Coordinator, within our Care Coordination department, will support clinicians within the section of Hospital Medicine as they provide services to patients hospitalized at the medical center. As a member of the multidisciplinary team, this position will support the clinical staff to promote timely patient care, enhance the communication with families, other providers, and external organizations. The Patient Navigation Coordinator will improve the transition of care process and enhance compliance with payer related tasks. This position will support all hospitalist providers on day services, patients and families, case management, and social work assigned to the hospitalist team. This position will also identify areas for improved efficiency of administrative processes.

Requirements

  • Proficient using the latest versions of Microsoft Office Suite and web searches.

Nice To Haves

  • A Bachelor’s degree from an accredited academic body is Preferred.
  • Experience working in a healthcare setting and supporting the patient experience is highly preferred.

Responsibilities

  • Support clinicians within the section of Hospital Medicine as they provide services to patients hospitalized at the medical center.
