Requirements

  • One year recent acute care experience required.
  • RN with 3 years of clinical experience in hospital setting with at least one year to include emergency and/or critical care setting.
  • Background in Utilization Management and a working knowledge of health care reimbursement.
  • Background in Neurology/Neurosurgical and Cardiology.
  • Experience using electronic bed management systems, preferably TeleTracking.
  • Experience in Patient Flow management.
  • Basic computer skills required.
  • Competent in select psychomotor skills.
  • Knowledge of current nursing technique, principles and practice.
  • Working knowledge of computer programs such as word, excel and outlook.
  • Current AZ State RN license in good standing required.

Responsibilities

  • Coordinates the transfer center activities for Dignity Health facilities in collaboration with hospital administration, nursing leadership or their designee, medical staff, physicians and transportation vendors.
  • Functions as the liaison between referring hospitals into and between Dignity Health facilities to coordinate the transfer and admission of patients to hospitals.
  • Participates in all activities, meetings, calls, and education to facilitate transfers from all points of entry to the acute hospital setting.
  • Bases facility designation on diagnoses, planned procedure and level of care requirements and acuity.
