About The Position

This position is required to be onsite at our 3033 North 3rd Ave location. As our Patient Logistics Coordinator, you will orchestrate seamless patient movement, optimizing bed utilization, coordinating transfers, and ensuring efficient patient flow. Your role is pivotal in minimizing delays and enhancing operational efficiency. Every day you will manage and prioritize patient admissions, discharges, and transfers based on medical necessity and bed availability. You will proactively communicate with clinical teams, discharge planners, and external facilities to resolve logistical challenges and ensure a smooth continuum of care. To be successful in this role, you need strong clinical judgment, exceptional organizational skills, and the ability to make rapid, informed decisions in a dynamic environment.

Requirements

  • High School GED, upon hire or High School Graduate, upon hire
  • One (1) year experience recent experience in an acute care setting
  • Two (2) years recent experience in a health care setting

Nice To Haves

  • Associate Of Arts, upon hire or Associates Of Science
  • Two (2+) years Medical Assistant or Nurse Assistant working in an acute care hospital setting

Responsibilities

  • Manage and prioritize patient admissions, discharges, and transfers based on medical necessity and bed availability.
  • Proactively communicate with clinical teams, discharge planners, and external facilities to resolve logistical challenges and ensure a smooth continuum of care.
  • Facilitate the placement of patients as directed by the Patient Logistics Manager, Patient Logistics RN, and physician orders.
  • Facilitate the transfer of patients to the appropriate facility and unit.
  • Identify potential problems and bring them to the attention of the Supervisor, ANS/House Supervisor, and/or Leadership to prevent and/or resolve barriers to timely placement and/or transfer of patients throughout the acute care setting.
  • Communicate with referring and receiving facility staff to ensure timely placement of patients to the appropriate level of care and unit.
  • Document activities clearly and accurately to promote effective communication with members of the care team, including Transfer Center, Hospital EDs, Hospital Units, Leadership, sending and receiving facilities or other providers.
  • Use effective communication skills with colleagues, customers, physicians, and care team members, both internal and external to facilitate the timely placement of patients.
