About The Position

Accompany Health is seeking a Patient Engagement Coordinator to play a central role in ensuring patients with complex needs receive consistent, high-quality engagement and support for their ongoing care. This role involves reaching out to existing patients to schedule Annual Visits, follow-up appointments, provide reminders, answer questions, and reinforce the value of continuous support. The primary focus is to help patients stay engaged with their care teams and build stronger relationships. This is a high-volume outbound call role, requiring the majority of the workday to be spent on back-to-back outbound calls, with a target of 135+ calls per day. Success depends on maintaining energy, focus, and warmth throughout the day, delivering an outstanding patient experience, ensuring continuity of care, and maintaining accurate records.

Requirements

  • High School Diploma/GED or higher
  • 2+ years of related experience in a high-volume outbound call environment, preferably in healthcare customer service or healthcare sales/direct marketing
  • Excellent patient communication skills, with the ability to quickly build trust and rapport with seniors and other patients
  • Strong computer skills and basic knowledge of G-Suite (e.g., Google Docs/Sheets)
  • Can handle multiple tasks at once, such as typing notes while talking and working in 2 systems at the same time
  • Applicants must complete an internet speed test as the first stage of the hiring process to ensure their connection meets performance standards.

Nice To Haves

  • Experience working in an Electronic Medical Record (EMR) system
  • Knowledge of Medicaid and/or Medicare programs and senior services
  • Ability to work after hours and weekends if needed

Responsibilities

  • Conduct outbound calls, texts, and other virtual outreach to schedule Annual Visits and follow-up appointments with existing patients
  • Maintain a daily outbound call target of 135+ calls, working through structured call queues in our internal CRM tools throughout the day
  • Send SMS and message-based outreach to patients to confirm appointments and follow up on scheduling
  • Navigate and maintain records across internal CRM systems and spreadsheets simultaneously during and after calls
  • Educate patients on the importance of ongoing care and the benefits of completing their annual visits and any necessary follow-up appointments with clinicians
  • Handle calls from patients with warmth and professionalism, answering questions and directing them appropriately
  • Collaborate closely with clinical care teams to ensure alignment on visit readiness and follow-ups
  • Provide an empathetic, respectful, and encouraging patient experience on every call
  • Monitor scheduling metrics and contribute to achieving engagement goals, maintaining both call volume targets and service quality
  • Act as a liaison between patients and the care team to address basic concerns and questions about services
