About The Position

Be a part of a world-class academic healthcare system, UChicago Medicine Medical Group, as a Patient Coordinator at our Hinsdale clinic. Here, you will work with our Multi-specialty Medicine team. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area. Our new UChicago Medicine Hinsdale location at 11 Salt Creek Lane builds upon the services offered at our previous outpatient center at 12 Salt Creek Lane. This state-of-the-art facility provides comprehensive multispecialty care for adults, including urogynecology and pelvic reconstructive surgery, gynecology, gastroenterology, ENT, transplant, neurology, and other services. We also offer specialized care for children and adolescents, including on-site services in gynecology, gastroenterology, neurology, dermatology, and cardiology. The Patient Coordinator serves as first point of contact for patients and providers to coordinate services with UCM medical professionals, ancillary services, BSD and hospital staff. They provide administrative support and customer service to facilitate efficient workflows for patients to access service across the network. Additionally, they give guidance to patients through an evaluation process, work with various departments to ensure patients complete their itineraries, revise and communicate itinerary changes as required.

Requirements

  • High school diploma or equivalent

  • Medical terminology certification
  • Bachelor’s degree, 2 years’ experience in hospital or medical office
  • Microsoft, Epic, Appointment Scheduling, Insurance Verification, Pre-certification, Prior Authorization, billing & collections, excellent verbal and written communication

Responsibilities

  • Serves as primary non-clinical contact for care coordination for patients, families and referring providers
  • Coordinates, schedules and manages appointments for consultation, education and testing
  • Liaises with clinical and non-clinical teams to confirm and validate schedules and ensure patients are on schedule throughout their visit
  • Provides advance notice to patients and referring providers of required follow-up testing and coordinates the care as needed
  • Collaborates with various departments to ensure billing information is accurate and current
  • Distributes program materials, welcome packets, educational materials, itineraries and other collateral as needed
  • Coordinates pre-visit assessments/planning/questionnaires
  • Facilitates required referrals, pre-certification, and authorizations as required
  • Screens patients for supportive care services if needed (i.e., dietician, social work and financial counseling)
  • Directs patients and families to available community resources and supportive services
  • Assists patients to overcome barriers to care, i.e., transportation, childcare, financial counseling
  • Performs other duties assigned
