About The Position

DaVita Kidney Care is expanding its services by bringing industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services (DSD). This initiative aims to reduce the burden on patients who are transported for dialysis three times a week and to decrease avoidable healthcare costs. DSD Services teammates will leverage DaVita's resources to develop and grow this new service line. The ideal candidate will have significant experience within DaVita, thrive in a start-up environment, be passionate about improving patient lives, and demonstrate a track record of getting things done. Teammates must be comfortable working autonomously and collaborating with cross-functional resources across the Village. In this role, you will take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs. You will work three days a week with no required on-call, but your impact will be significant. DSD Dialysis care is a complex but fulfilling service line where meeting and/or exceeding clinical outcomes are the goals. Multitasking is expected to meet the needs of the service, patients, and SNF partners. You will provide comprehensive patient care in an independent setting, requiring constant critical thinking and application of core skills to monitor patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup. Requirements

  • Prior Hemo-Dialysis experience
  • Valid Certified Clinical Hemodialysis Technician (CCHT) certification
  • High school diploma or equivalent
  • Basic computer skills and proficiency in MS Word and Outlook

Responsibilities
